  Form #02.001 – AffCitDomTax

  Dear SEDM Team,

   

  The electronic fillable AffCitDomTax form does not work properly.   The tab key moves the cursor to the various links on the page but not to the various electronic fillable fields.    Please fix the form and Re-post it for your member’s use.   Thanks for your help.

  I believe there may be an error (please correct me if I am wrong) on page 11 of this form in the perjury statement.  The form says “Remedy for perjury may only be pursued in a state (and not federal) court under the common law and statutory civil law. ”  Should this read “…and NOT statutory civil law”?
